Access to The Link will be via referral. Each referral will be considered by the Inclusion Team to understand the needs of the pupil and what actions have been taken by school.
Outcomes of the referral could include:
1. Recommendations of actions to be completed by school
2. Offer of an outreach visit from a Specialist TA
3. An intervention block provided by a Pastoral Support Worker – working together with a member of school staff
4. Offer of a placement at The Link
The Link will provide:
· Visit from ‘The Link’ staff prior to starting the placement including individualised resources to prepare pupils for starting
· Assessment of pupil’s needs to enable planning of bespoke intervention and to review progress
· Each pupil will have an allocated ‘Link key worker’ to ensure consistent communication between The Link, school and families
· Weekly updates on progress
· Termly review meetings
· Outreach visits from ‘Link Key worker’ to support transition between settings
· Bespoke CPD for staff (through a CPD at school, and on-site sessions for staff)
· Planned return to school with a phased transition
What the school will do:
· Attend a visit prior to the pupil starting at The Link to support transition
· Arrange transport
· Communicate weekly with ‘Link key worker’ updating on observations and progress in school
· A consistent member of staff who will be the pupil’s 1:1 at The Link– this is key to the progress of pupils. It supports the link between school and provision, and enables the development of staff skills.
· Embed the recommendations provided by The Link
· Participate in review meetings
· Support communication with families
